PCNL : Percutaneous Nephrolithotomy (Keyhole Kidney Stone Surgery)

Percutaneous Nephrolithotomy (PCNL) is the internationally recognized gold-standard procedure for the removal of large, complex, or hard kidney stones (typically greater than 2 cm or 20 mm in diameter), including staghorn calculi.

Instead of open surgery that required a large 15–20 cm incision and weeks of bed rest, PCNL is performed through a tiny "keyhole" access (approx. 1 cm) created in the patient's flank directly into the kidney. Under high-definition endoscopic and fluoroscopic guidance, advanced lithotripsy devices (ultrasonic, pneumatic, or laser) break the stone and continuously extract all fragments, providing the highest single-stage stone clearance rate.

Key Advantages of PCNL

  • Highest Stone Clearance Rate: Over 95% single-session success rate even for very large and complex stones.
  • Minimal Keyhole Access: Replaces massive open surgical cuts with a single 1 cm puncture on the back.
  • Direct Stone Removal: Stone fragments are actively extracted and suctioned out, rather than left to pass painfully through the urinary pipe.
  • Safe for Obstructed Systems: Ideal when the ureter below the kidney is narrowed or obstructed (e.g., PUJ obstruction) where RIRS cannot reach from below.

Every kidney stone is unique. The choice of treatment depends on the size, hardness, location of the stone, and the patient's individual anatomy. Below is a comprehensive comparison of all modern stone treatments available at Dr. Chirag Urology:

How Dr. Chirag Recommends the Right Procedure For You

To determine which surgical option guarantees highest stone clearance with the fastest recovery, a thorough diagnostic assessment is conducted:

  • Non-Contrast CT KUB (Gold Standard): Measures stone size in millimeters, exact 3D location, and stone hardness (Hounsfield Units - HU).
  • Stones > 20 mm: Standard PCNL is strongly recommended as it extracts the mass in one setting, avoiding multiple procedures.
  • Stones 10 mm – 20 mm: Either RIRS (no external cut) or Mini/Micro PCNL (pinhole approach) are preferred based on stone density and lower pole calyx anatomy.
  • Stones < 10 mm: ESWL shockwaves or medical expulsive therapy if non-obstructive.
  • Stones in the Ureter: URS with laser fragmentation for quick obstruction relief and kidney protection.
